SETTLEMENT OF THE CLASS ACTION REGARDING NEIGHBORHOOD DISTURBANCES IN THE CITIES OF LA PRAIRIE AND CANDIAC, FROM 2010 TO 2016
INSTRUCTIONS TO READ BEFORE COMPLETING THE CLAIM FORM
This class action concerns:
1. Any natural person, including minors;
2. Residing or having resided in sectors “A” and “P” of the City of Candiac and in the sector known and designated as Faubourg du Golf de La Prairie;
3. Between February 12, 2010, and December 31, 2016.
The street addresses of the residences concerned this agreement are listed in the Distribution Plan, which you can consult here.
The deadline for submitting a claim is July 21, 2026.
IMPORTANT : all residents of an eligible address must submit their own claim form.
Therefore, 2 residents = 2 claims.

Your personal information is collected, used and retained by Class counsel and Proactio in accordance with applicable privacy laws and regulations:
- for the purposes of operating and administering the Settlement Agreement in this class action;
- to assess and review the claimant's eligibility under the Settlement Agreement; and
- are strictly private and confidential and will not be disclosed without the express written consent of the claimant, except as provided in the Settlement and Compensation Agreement.
